 Tehama County Health Partnership 

The Partnership's Vision, Mission, and Scope is:

To develop a multi-disciplinary approach addressing the broader determinants of population health; thus redefining the concept of health to include not only physical and mental health, but also education, economy, safety, housing, transportation, recreation, and other aspects that influence our quality of life.

The Partnership is involved in several policy and client level activities to make this vision a reality. The Partnership is formalizing its procedures related to planning, resource allocation, service delivery, communication, and evaluation. The Partnership has the following three-part mission which has served to focus its activities during the HII planning period:

To collect existing information and create on-going assessments of:

  1. current resources, services, and relationships among providers and clients;
  2. the population health needs of all residents in Tehama County;
  3. service implementation;

To develop a self-sustaining, coordinated effort to improve the health and well-being of Tehama County residents through the establishment of integrated systems of prevention services that address the psycho-social, economic, environmental, and cultural factors related to health;

To develop a detailed community health improvement plan that

  1. institutionalizes prevention efforts;
  2. creates new and expanded preventive health services;
  3. reallocates and restructures existing resources;
  4. makes changes in ineffective or detrimental policies and practices;
  5. creates process and summative evaluation methods;
  6. develops a seamless, interagency collaboration model that addresses the broader determinants of community health leading to measurable changes in the quality of living of Tehama County residents.
